Informed Consent, Waiver, and Release Agreement

I freely enter into this Waiver and Release with FIRE OF LIFE Workshop, its instructors, affiliates, landlords, and executors (collectively “The Workshop”).  I hereby release The Workshop from all liability with respect to any accidents that might occur.  I also understand that The Workshop does not provide accident insurance.  I am satisfied that all premise conditions are reasonably safe for my participation. 

1.  I understand that to receive a full refund, I have up to 14 calendar days prior to the workshop start date, to notify The Workshop in writing and after that time expired, there is a $250 non-refundable deposit. 
2. I consent to any employee or other personnel affiliated with The Workshop, to seek medical attention and treatment or other measures deemed necessary or advisable at the discretion of The Workshop for myself in the event of an accident, sudden illness, or other condition that occurs during The Workshop. 
3. I understand that The Workshop will make reasonable efforts to contact my emergency contact in the case of an accident, sudden illness, or other condition.  I release The Workshop from and of any liability for decisions or actions in seeking medical care, and I agree to pay all the costs and fees for the medical care or treatment authorized under this emergency medical authorization.
4.  I acknowledge that the recreational activities during breaks, swimming, field trips, transportation, and physical activity carry with them the potential for damage or loss of property, serious injury, or death; that I assume the risks of participating in these types of events and activities; and that I am fit for participation.
5. I acknowledge that if I am unable to follow the rules of The Workshop that should I be asked to leave The Workshop at the sole discretion of The Workshop’s instructors, I may be asked to leave The Workshop and that there shall be no refund of the fee or of any part of the fee for participation in The Workshop.

By checking this box:

• I agree I have entered into this agreement freely and voluntarily without force or coercion.
• I agree to these terms and I expressly waive, release, and discharge from any and all liability for, and indemnify, hold harmless, and promise not to sue for, death, disability, personal injury, or action of any kind, The Workshop, its instructors, affiliates, and employees as a result of my participation in said activities.
